The Horizon IT system, developed by Fujitsu, falsely reported financial shortfalls at post offices nationwide. Despite knowing about systemic flaws, both the state-owned Post Office and Fujitsu continued prosecuting innocent subpostmasters for theft and fraud. Victims were forced to repay nonexistent debts, with total taxpayer costs reaching €700 million.

The report details how wrongful accusations destroyed lives: families lost homes, victims faced social ostracization, and mental health crises proliferated. Campaigner Jo Hamilton described the findings as showing "the full extent of the horrors they exposed us to."
